ATTENTION BAND PARENTS! SEE BELOW FOR ADDITIONAL IMPORTANT INFORMATION

ATTENTION

All 9th-12th graders MUST have a completed (passing) Sports Physical and Medical History Form to participate in the band program by May 20th.

Graduating Seniors (Class of 2022) going to Washington DC must have a NEW Medical History form completed by May 20th.

Please go to the following link to find the UIL Physical Form, Medical History Form, and the REQUIRED CONSENT FORM if your child is getting a physical on their campus in Spring 2022.

These are the following forms to be used for registration. Forms will be provided on Registration Day.

Student Travel Release must be completed 4 days before an event. Release requires signature from the principal.
